Oxford's Sweat Equity Program (previously known as the Gym Reimbursement program) offers reimbursement for exercising an average of two to three times per week - up to $200 every six months. Covered spouses can receive up to $100 reimbursement per six-month period. In addition to facilities promoting cardiovascular wellness, you may now earn reimbursement for participating in classes ranging from aerobics, Pilates, and yoga, to boot camps and weight resistance training. Individuals can combine programs from a variety of different facilities and fitness classes to reach the required 50 classes and/or visits needed in each 6-month period to receive the maximum reimbursement.
Here’s how the Gym Reimbursement Program works:
Visit the gym or attend exercise classes a minimum of 50 times per a six-month period. Reimbursements will not be issued until six months have passed, even if 50 visits are completed sooner than six months.
Collect paperwork including a copy of your current gym/exercise class bill showing the monthly cost of your membership and proof of payment for the six-month period you are submitting for reimbursement.
Complete the Sweat Equity Program Reimbursement Form.
Mail the Gym Reimbursement Form along with the copy of your current gym/exercise class bill and proof of payment within six months to:
