Reminders

1. Caremark is your prescription drug care provider as of June 1, 2006. Click here for a list of phone numbers and addresses.

2. Decision Power  offers participants the most appropriate and cost-effective medical care possible by addressing each participant's individual needs. It covers chronic disease management and preference-sensitive condition management. This program provides clinical and financial modeling capability, health coaching/member outreach, and physician communication. This year, Decision Power has set goals to integrate with all medical management intervention, leverage any interaction to "case find", improve clinical performance of participating providers, and improve member well-being and medical outcomes. Please call
(800) 893-5597 24 hours a day, seven days a week to speak to a Decision Power
Health Coach. This service is offered to Fund participants at no cost.

3. MultiPlan is a network for out-of-network providers who agree to provide services at favorable rates. The rates effectively lower the amount of coinsurance you pay. The program doesn't include chiropractic, mental health, lab tests, or other ancillary services.

A list of MultiPlan providers can be found by calling (800) 672-2140 or by logging on to www.multiplan.com.

4. Locate an MHN mental health provider online at www.mhn.com. Select "Members" on the home page, then choose "Practitioner Search."

5. Examine your vision benefits eligibility on Davis Vision's Web site. Sign in using "Member Login" and choose "Check Eligibility."

6. Health Net updates its provider directory every week. You can search for providers by specialty, location, or gender.

7. Student letters are due twice a year: by Jan. 1 for the spring semester and by Sept. 1 to ensure coverage through Dec. 31.

The original letter from the school registrar should include your and your dependent's Social Security numbers and verify that he is a full-time student.

8. Quest is the only in-network lab. Locate Quest Labs by calling the Health Net Directory by Phone at (800) 686-9847 or checking online at www.healthnet.com.

9. You have two Benefits Fund identification cards. One is for medical care coverage through Health Net. The other card is for dental through Aetna and prescription drug coverage through Caremark.
